1 Jul 06 through 30 Jun 07 Premiums are monthly charges and include the cost of health, dental and vision coverage. Benefit Recipients and/or Dependent Beneficiaries who enroll in a managed care plan will pay lower monthly premiums (deducted from SURS annuitant's check). NOTE: Percentages listed in each row/column reflect premium increases (or decreases) for 1 Jul 06 through 30 Jun 07 in comparison to premium charges for 1 Jul 05 through 30 Jun 06!
